Abstract:
Objective. To monitor the dynamics of biochemical markers of inflammation and indicators of brain tissue damage in the acute period of ischemic stroke (IS) while taking submaximal doses of Mexidol. Material and methods. The study included 60 patients admitted within the first 6 hours after the onset of the disease, with verified IS according to the ASPECTS scale of 7-9 points. Patients of the main group (n=30) received submaximal doses of Mexidol - 750 mg of the drug diluted in 250.0 ml of saline, 2 times a day intravenously by drip for 6 days. Patients of the control group (n=30) were prescribed standard therapy. All patients underwent determination of biomarkers, assessment of neurological status within 60 minutes from the moment of admission to hospital and on the 7th day. Results and conclusion. In both groups, C-reactive protein, interleukin-6, and fibrinogen levels changed insignificantly and were not significantly relevant to assessing Mexidol's efficacy. Biomarkers such as brain natriuretic peptide and pregnancy-associated plasma protein-A, as well as average NIHSS and MFI-20 scores, significantly decreased in the Mexidol group compared to the control group. These results demonstrate a reduction in oxidative stress and signs of inflammation in the acute phase of ischemic stroke with submaximal doses of Mexidol, a decreased severity of neurological deficit, and an increase in patients' physical activity. Given the timing of activation of the body's damaged antioxidant system, the recommended duration of drug therapy is 3 months.
