The article of a review nature outlines the issues of pathogenesis, clinic, diagnosis and treatment of chronic brain ischemia. The aspects of the development of "disease of small vessels", endothelial dysfunction, processes of peroxidation of lipids, apoptosis and neuroplasticity are highlighted. It was noted that the joint management of patients with chronic brain ischemia neurologists and doctors of other specialties, the appointment of individualized pathogenetic therapy.
