This review article discusses the pathogenesis, clinical presentation, diagnosis, and treatment of chronic cerebral ischemia. It covers aspects of the development of small vessel disease, endothelial dysfunction, lipid peroxidation, apoptosis, and neuroplasticity. It emphasizes the need for collaborative management of patients with chronic cerebral ischemia by neurologists and physicians of other specialties, as well as the prescription of individualized pathogenetic therapy.
